ORQA Releases Free Risk Management Virtual Appliance PDF  | Print |  E-mail

ORQA is pleased to announce the release of a Virtual Appliance to allow for quick and simple implementation of Active Agenda's Operational Risk Management server application with Healthcare extensions by ORQA, LLC.  Special thanks to Active Agenda, Inc, and TurnkeyLinux for making this possible.

Active Agenda is an Operational Risk Management Web 2.0 application that has over 100 modules ranging from policy management, fleet management, OSHA injury reports, risk assessments, root cause analysis, KSA's, and many, many others.  ORQA has extended Active Agenda's functionality in to healthcare to provide the first Open Source patient safety/healthcare event reporting system.  We have also included peer review and technologist feedback modules as examples to demonstrate how Active Agendas functionality can be customized for healthcare settings.

Far beyond a simple web-based event reporting solution, accessing our Web 2.0 application does not require Java, ActiveX, or other plugins and our reporting modules can be integrated with point-of-care applications for simple reporting from end users where the events occur.  Once the reporting data is collected, follow up activity including risk assessment, RCA, corrective actions, learning events, policy changes, and formal guidance can all be associated with the original reports, to establish continuity throughout the continuous improvement cycle.

This has been released under the Reciprocal Public License which is freely available and open source, but differs from the GPL in that if you modify or extend the application and use in a commercial or public setting, you must "reciprocate" and make the changes available freely to the community.

This is a fully-functional server, and we highly encourage exploring as many modules as possible.  However, to allow for "plug & play" type functionality so it immediately works as a virtual appliance, the system is not optimized for performance or security and therefore recommend significant modifications before using in a production environment.

ORQA FORMS PATIENT SAFETY ORGANIZATION

Peoria, IL—May 4, 2009— ORQA, LLC is listed as a Patient Safety Organization (PSO) by the Agency for Healthcare Research and Quality (AHRQ), on behalf of the Secretary of the U.S. Department of Health and Human Services.

PSOs are designed to help clinicians, hospitals and health care organizations improve the care they deliver to patients by encouraging them to conduct quality and safety analyses. Through federal protections of legal privilege and confidentiality, which are authorized by the Patient Safety and Quality Improvement Act of 2005, PSOs foster a culture of safety and create a secure environment where providers can collect and analyze data to identify and reduce the risks and hazards associated with patient care.

“ORQA was founded to eliminate the technical barriers limiting clinician participation in patient safety,” said Mike O’Meara, ORQA’s Founder and President, “but only through the protection offered by being listed as a PSO have we been able to address concerns about disclosure of data and liability. ORQA’s listing as a PSO is a perfect fit with our software and service solutions designed for today’s complex environment of care.”

PSOs also are designed to aggregate data across multiple health care provider groups, which will help identify issues quickly and allow improved early warning and communication about ways of reducing risk and improving quality. As de-identified data become available from PSOs, a Network of Patient Safety Databases will receive, analyze, and report trend data nationally which will be included in AHRQ’s annual National Healthcare Quality Report.
